Please use the search tool below to look up the certification status of individuals or individuals associated with churches.

Search by church

• Select your Church Name from the drop box
• Click the "Search" button 

Search by individual

• Fill in the first and last name fields; please use proper names in the first name field. You can also enter partial name information.
  Ex: If searching for Jane Doe, you could type in first name: Ja, last name: Do
• Click the "Search" button
• You may remove a Church Name by selecting the blank at the top of the drop down menu

The Form Missing section lists the most up to date information we have, but information changes daily.  We may have received forms that have not shown up in our system yet.
